Explicit Consent for Minors Policy

EarthWorks ClaySpace requests permission to capture and use media (photos, videos, or other recordings) featuring your child while participating in activities at our studio. These media assets may be used for purposes such as:

Social media platforms (e.g., Instagram, Facebook, TikTok)
Website galleries, blog posts, or promotional pages
Printed materials (e.g., flyers, posters, event promotions)
Digital advertising

By signing this form, you acknowledge and agree to the following:

I consent to EarthWorks ClaySpace capturing, using, and publishing media featuring my child for the purposes outlined above.
I understand that my child’s name will not be used in connection with any media without additional specific consent.
I acknowledge that EarthWorks ClaySpace retains ownership of all media captured and reserves the right to edit or adapt content as needed.

I, the undersigned parent or legal guardian of the minor(s) named above, have read and understand this Media Release Form. I give (or deny) permission as indicated by my selections above.

Your child will be excluded from all photos and videos taken within the studio.

I hereby give my consent to use my photographs according to the Terms & Conditions.
